No patient wants to be readmitted to the hospital once he or she has been discharged. Yet as emergency care providers, we bring a lot of the same patients back to the hospital time and again for problems which might have been prevented.
That’s why we launched the Community Paramedicine program for patients who are at high risk for readmission. Within one to two days of discharge, Burholme paramedics visit them at home, complete a medical and home risk assessment, review medications and discharge instructions, and encourage them to stay on track with their recovery plan.
After 180 days with the program, we’ve seen the readmission rate for this group drop from 18 percent to just 5 percent!
Funded by the Albert Einstein Society, this partnership between Einstein and Burholme Emergency Medical Services is a cost-effective way to limit unnecessary readmissions and support healthier patients. No other hospital in the area is doing this, and it’s all possible thanks to the Einstein Society.
