The care we provide our elderly patients is critical. Due to the frequency and length of their average stay, ideally, these patients should have a care provider discuss treatment and review medications, as well as offer social support to them and their families, both during their time at Einstein and after discharge.
We applied for a grant from the Albert Einstein Society to fund Nurses Improving Care for Healthsystem Elders (NICHE). NICHE grew out of the need to provide support to a vulnerable group of patients. If our grant is approved, we can work to ensure all patients 65 and older receive sensitive and exemplary care from geriatric care professionals. The program will help us identify at-risk patients and put plans in place to enhance their treatment, both here and after discharge.
It’s easy to fall in the trap of doing things the same way all the time, but having access to apply for grants through the Albert Einstein Society is allowing us to change the culture of patient care. The Einstein Society is opening a whole new world for us.
